This Report Template supports programs and services to capture and share the results of a completed outcomes survey using the Community Services Outcomes Tree. The template provides a structure through which to report important, accessible and meaningful data. The template can be easily adapted to individual service/program contexts.
The template provides instructions and prompts to facilitate its completion.
